Parade Participation & Liability Waiver

By registering for and participating in any parade program, rehearsal, class, or performance with ALZA Dance Co. LLC (“ALZA Dance Co.”), I acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in dance activities, including but not limited to Zoom rehearsals, in-person rehearsals, performances, and parades, involves inherent physical risks. These risks may include, but are not limited to, muscle strains, sprains, falls, collisions, dehydration, illness, or other injuries.

I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation, whether occurring during virtual instruction, in-person rehearsals, travel, performances, or parade events.

2. Release of Liability

I hereby release, waive, and discharge ALZA Dance Co. LLC, its owners, directors, instructors, employees, contractors, volunteers, and representatives from any and all claims, demands, causes of action, or liability arising out of or related to any injury, illness, loss, or damage sustained while participating in ALZA Dance Co. activities, including those conducted virtually via Zoom.

This release applies to injuries caused by negligence to the fullest extent permitted by law.

3. Medical Responsibility

I confirm that I am physically able to participate in dance activities and that I have no medical condition that would prevent safe participation. I understand that ALZA Dance Co. LLC does not provide medical insurance or medical care.

I agree that I am solely responsible for any medical expenses incurred as a result of participation.

4. Participation Requirements

I understand that parade participation is contingent upon meeting all program requirements, including but not limited to:

• Purchasing the required costume by the stated deadline (or using an approved dance team uniform)

• Attending the required in-person rehearsal(s)

I acknowledge that failure to meet these requirements will result in ineligibility to perform, without refund.

5. Refund & Cancellation Policy

I understand that parade registration fees are partially refundable only if cancellation is submitted at least 24 hours before the first scheduled class or rehearsal.

After that time, no refunds will be issued for any reason, including but not limited to scheduling conflicts, illness, injury, or failure to meet participation requirements.

6. Media Release 

I grant ALZA Dance Co. LLC permission to use photos and video recordings taken during rehearsals, performances, or parades for promotional, educational, or marketing purposes, without compensation.

7. Acknowledgment & Agreement

I have read this waiver carefully and fully understand its contents. I understand that by agreeing to this waiver, I am giving up certain legal rights.

I acknowledge that my participation is voluntary and that I agree to all terms outlined above.
